Financial Policy

We participate with many, but not all insurance plans.  We will file claims for Medicare, Wellmark BCBS, United Health Care and employer groups with the Mercy PHO.  For all other insurance companies, we will provide a completed bill for you to file your claim. 

We will ask to see your insurance cards at each visit. 

Your payment for all copays, refractions, and other non-insurance covered services is due at the time of service. 

If you have an insurance plan that requires a referral, it is your responsibility to obtain that from your family doctor before you come in for your appointment.

If you have any questions regarding charges or insurance, or if you are concerned about payment, please consult with our insurance department.   For your convenience, we accept Visa, MasterCard, Discover Card, personal checks, debit cards or cash payments. 

 

Confidentiality

In an effort to protect your privacy, we will need your signature to authorize the release of any medical records or information needed to file your insurance claim.  We may call you to remind you of an upcoming appointment and may leave a message on your answering machine regarding that appointment.
